Overview
Custom orthotics are shoe inserts made to match the shape and mechanics of your feet, rather than a one-size-fits-all insole from a shop.
They support the arch and control how the foot rolls and takes load as you stand and walk. Because the way the foot meets the ground affects the ankles, knees, hips and lower back, they are often considered for problems linked to foot posture, such as plantar fasciitis and painful flat feet.
Orthotics are one tool among several. They usually work best alongside exercises for the feet and legs, suitable footwear and changes to activity, and not everyone with flat feet needs them.
Who it’s for
Dr. Ho checks whether it is suitable for you at your first visit.
It may suit people with
- Plantar fasciitis or heel pain linked to foot posture
- Flat feet or arches that roll in, with foot, ankle or knee pain
- Front-of-knee pain where the way the foot rolls adds to the load on the knee
- Foot or shin pain with running or long hours of standing
- A high, rigid arch that absorbs shock poorly
They are usually unnecessary, or need extra care, if you have
- Flat feet with no pain or other problems, including flexible flat feet in children
- A recent injury that needs other treatment first, such as a suspected fracture
- Diabetes or reduced feeling in the feet, where the fit must be checked carefully to protect the skin
- Shoes with no room for an insert, such as some slim fashion shoes
Dr. Ho will tell you honestly if orthotics are unlikely to help you, and suggest other options.
What to expect
General information only — your exact treatment is set after an in-person assessment.
Assessment: Dr. Ho looks at your feet, arches and footwear, checks how your ankles, knees and hips move, and watches you stand and walk.
Capturing your foot shape: Your feet are measured and their shape is captured, so the orthotics match your arches.
Making: Your orthotics are made to your measurements and the correction your assessment calls for. Dr. Ho will tell you how long your pair will take.
Fitting and review: At the fitting, the orthotics are checked in your shoes and adjusted if needed, and you are shown how to wear them in.
Wear them for an hour or two at first and build up over one to two weeks. Some new awareness under the arch is normal; pain or rubbing isn’t, so let us know and they can be adjusted.

Good to know
Orthotics change how your foot takes load while you wear them; they don’t permanently change the shape of your foot.
What it may help with
- May ease foot, heel and knee pain linked to how the foot rolls and takes load
- Support the arch during long hours of standing, walking or running
- Make it easier to stay active while other treatment and exercises take effect
What it can’t do
- They don’t permanently reshape the arch or "cure" flat feet
- They don’t replace exercises to strengthen the feet, ankles and hips
- They aren’t needed for flat feet that cause no problems
FAQ
Shop insoles come in standard shapes and mainly add cushioning or general arch support. Custom orthotics are made to your foot shape and to the correction your assessment shows you need, so they can control how the foot moves more precisely. For some people a good off-the-shelf insole is enough, and Dr. Ho will tell you if so.
Usually one to three years, depending on the material, your weight and activity, and how often you wear them. Have them checked once a year, and sooner if they look worn or your symptoms come back.
They fit best in shoes with a removable insole and enough depth, such as trainers and many work shoes. Slim dress shoes may need a slimmer design, so bring the shoes you wear most to your assessment.
Most people wear them in the shoes they use for most of the day, or for sport. How much you need them depends on your condition, and Dr. Ho will advise you.
Take them out to air after use, wipe them with a damp cloth, and keep them away from direct heat, such as a heater or a car in the sun, which can warp them.
